Merges master from dbushell/Pikaday. No new release at this time.

Couple minor tweaks and fixes.

Time Specific Changes:

• Feature : New option timeLabel: 'Time:' allows adding a label to time select
• Fix : If no date and time is set for a date picker, the time inputs will default to 0
• Fix : Handling of min and max dates. Closes #32
• Fix : Merged master of upstream. Closes #41
