Changes to glossary.md

Changed the glossary md formatting. Formatting changes are seperate from the actual content-changes so we can discuss or merge them seperatly

@JensPenny I'd like to review these - in the call or in preparation for it.
Mostly the definitions seem consistent but there are some definitions that alter the meaning I had intended - which is fine, just want to evaluate the impact with you.
The Medication Reconciliation is good and it is a functionally "loaded" definition - it implies that the Medication Record/Scheme/List is not just a passive view of the medication, but an active driver of the treatments - which I think is what is intended but we should discuss.

We should also think about the Medication Administration Request, which is a concept that is not frequently used in a hospital context (where the prescription is generally considered to be the administration request), but is used in a Home Care context. An MD can request another caregiver to administer a drug: there will be a lot of commonalities between a prescription and a MAR, but they are functionally different (MAR does not imply a request to deliver the medication by the pharmacy), and this may result in different needs for medication description.
