Incorrect access url in sub org MyAccount apps

Description

In shared myaccount apps, access url does not contain a "/" between org Id part and myaccount part.
Example:
https://localhost:9443//t/carbon.super/o/a44b5219-3d09-4733-bc9b-2d02a6b5493bmyaccount

Steps to Reproduce

1. Get application details of my account using Application management API.
2. Observe the access URL.

"/" is missing between org Id part and myaccount part.

Due to this, My account redirection after password reset is also not working for sub org users.
