Data: Add new data fields in preparation for full data export

We need to add a few data fields to prepare for the full export from the COPA preschool application system. We are still waiting on an example data set from our partners.

There are a few outstanding clarifying questions about the data fields, I've added those here but they are marked with a (?). I will follow-up with our partners specifically on these items.

• Add (actual) neighborhood name field Data: Add neighborhood name field #928 (note: current neighborhood_name is actually community area name)
• Add neighborhood ID (note: current neighborhood_id field is actually community area id)
• Add "aka" name field
• Add "Include on Preschool Application" field
• Add "Last modified on" field
• Add an “Active/Inactive” site field
