tell client if writing to a closed pty

If the client tries to send keystrokes to a closed pty (i.e. if a session reconnected after a disconnect longer than the session timeout) send an event to the client that says it's a dead terminal - and process on the client to close the window (for fullblown ttyx - not the basic version)
