Ontologically, it is better to have the sections organized by topic rather than by +/- on a dimension when a subtle name change completely reverses it.

I'm not particularly concerned about underscores vs hyphens, but I think putting the main topic first and the modifier afterwards will help with file maintainability/augmentation.
Many of the questions appear in two list. This will keep those lists together if the file is alpha-sorted by category.
For example, we have a lot of polar opposites that share a most of their questions, just with ~opposite weightings.
Coffee-friendly vs Coffee-averse (rather than no_coffee).
Drug-friendly vs drug-averse
Tattoo-friendly vs tattoo-averse
Sports-friendly vs sports-averse
420-friendly vs 420-averse
Alcohol-friendly vs alcohol-averse

That said, it might not be easy to do for all of them. I'm not thinking of a great reversal for "non-spiritual."
"Spiritually-hostile" doesn't sound like it will go over very well, though maybe the atheists won't take much offense to "religion-averse."
