Fix headings on single clinic selection page

Description

In 89223 @JunTaoLuo noted the headings on the single clinic selection page are off.

• The facility name was coded as a heading but styled as bold text. It is now styled as a heading
• The phone label was coded as a heading but styled as bold text. It is now styled as a heading.
