Since indexing requires that we can ensure document elimination, in order to implement indexing lookups that we
will need to first parse the condition groups and then ensure that we have a covering index on each group.
Then we can search the indexes for each group to obtain a list of all possible document IDs,
then use those document Ids to early eliminate documents from the main lookup loop.
If any one condition group does not have an index, then no indexing will be used at all since all documents
will need to be scaned anyway. To prevent unindexed scans, reduce the number of condition groups.
All methods inside "management" namespaces should handle exceptions for logging and then throw the exceptions.
All API controllers (not the API management classes, but the service controller) should catch and handle exceptions. Do not throw them.
All long running processes should run Transaction.EnsureActive() often to throw an exception if the transaction has been killed.
