13% of explicit LSJ links into Perseus is dead (or not hyperlinked)

I've checked them all - yet only those entries that explicitly mention LSJ (and I may have omitted some)
375 entries are not found in LSJ; 13% of the 2,874

I've gone through the first two dozen and found 2 false positives. I've also found an entry (C8280) that's not even hyperlinked into Perseus - and perhaps it's useful to tell that all I have done is match the transcribed Greek in the CDO XML to that of the 1940 LSJ; the fact that I found this one is just pure luck - and only now I realise that you are using the 1968 version.
Having said that, I have clicked these entries in the CDO, and if they're not green they either return an error or have no hyperlink at all

Mm, I guess this is a slight misunderstanding (or misrepresentation on the CDO's part) of what these links are - they are just lookup links in Perseus, which don't guarantee that the item is actually listed in Perseus. Hypothetically we could use your list to remove links that are currently dead, but I'm not sure if that's a good idea, since Perseus could theoretically add them (and could even use the incoming dead queries to identify items that are missing), and conversely, they could remove items and we don't want to repeatedly query them to check.

So perhaps a better move is to change the caption or tooltip of these links to reflect what they are (currently the tooltip reads "Look up in Perseus", which was not meant to convey that the item was verified as matching there). Any suggestions on this are welcome.

The words are all there of course, in LSJ - most are just misspelled in the CDO, e.g. ἀγνεία is wrong whereas ἁγνεία is correct, and you can see how the spiritus lenis must be a spiritus asper, significantly changing the pronounciation of the word: cf. https://www.biblestudytools.com/lexicons/greek/nas/hagneia.html

Some are spelled correctly but pointing to the wrong entry, like http://www.perseus.tufts.edu/hopper/text?doc=Perseus%3Atext%3A1999.04.0057%3Aentry%3Da)%2Fdikos which is ἀδίκως, the adverbial form of ἄδικος and part of that last entry instead of being one itself, and ἄμυλον is identical in that regard.
What would seem the issue here is that only one entry is given, whereas two should be present: one that represents the coreect Greek word, and that is present in this case, and one that should point to the correct LSJ entry. I have done the same in my Translation by providing a transcription of the text and also a pointer to CDO: sometimes those are the same, sometimes they are not (and in the case of Thomas, very infrequently so)

What I am guessing that was done is that the Greek word was supplied (whether that was spelled correctly or not) and that lookups into LSJ were created that took the Greek, including diacritics, and converted those to lookup values accepted by Perseus because they complied with Perseus transcription rules, e.g. "a)/" for ἄ.
The reason that it now fails is twofold: the word was initially transcribed incorrectly, such as HAGIOS above, or it was not an entry itself but merely a subentry that was part of another, such as ADIKOS above. The first attests to a lack of knowledge of Greek (the example is very much like saying air instead of hair), the second to a design flaw
