This is extremely rare. I think it's only happened about twice in 10k hoards.
I was also not watching it when this happened so I do not know the exact steps that occurred but when outside HoH, the pathfind triggered for the (previous?) step in the ruby sea, thus causing you to go to the middle of the ocean and being stuck there.

so i just had this happen. he found the hoard and died to trap explosions and monsters on his way there.
On the scoreboard he tried leaving a bazillion times - then instead of queuing again he tried to run through the entire beach area until i stopped him manually.
Maybe add a Zone ID check resetting the plugin to "i need to queue" when zoneID is wrong in between refreshing navmesh coords where to walk to should fix that?
